CARE COORDINATION TIME
Coordination Services
Actual care coordination services are billed separately based on the time used.
The $99 monthly fee is the membership and access fee. Care coordination time is billed separately at $50 per hour.

For clients who would benefit from ongoing medication support, Divine Care Services offers a comprehensive monthly medication management package designed to help keep medications organized, current, and coordinated with healthcare providers and pharmacies.
The Medication Management Package includes weekly visits for consistent, personalized medication support.
